La Cattedrale è una delle più antiche chiese di Prato. Testimoniata già a partire dal X secolo come pieve di Santo Stefano, era la chiesa principale di Borgo al Cornio, il primitivo insediamento pratese. Ristrutturata dal X al XV secolo, appare unitaria per l'equilibrata purezza dei volumi e la vibrante bicromia dei paramenti esterni, in alberese e marmo verde di Prato (serpentino).
La struttura attuale risale al XII secolo (di essa rimangono le fiancate ed il campanile, tranne l'ultima cella costruita nel 1356). Nel corso del Trecento, per la crescente popolarità della reliquia della Sacra Cintola (in città dal 1141 secondo la tradizione), l'edificio venne allargato: dapprima fu costruito il transetto (forse su progetto di Giovanni Pisano), poi venne eretta la Cappella della Cintola. Gli edifici che si trovavano davanti alla facciata, inoltre, vennero demoliti per poter creare una nuova, larghissima piazza, per ospitare la folla in occasione delle ostensioni della sacra reliquia.
